 While utilizing your health insurance may be convenient and may be financially necessary, please be aware that using your insurance means you must have a mental health diagnosis and that information is reported to your insurance company. Your insurance company has the right to review your records at any time, and to ask for additional information regarding your care and our work together. Additionally, your insurance provider may determine how many sessions you get and the time of those sessions.  

 We accept Anthem BCBS, Cigna, Optum UBH Commercial Plans (UnitedHealthcare, Oxford, UMR. ConnectiCare, etc.), Husky A-D, Medicaid, Self Pay (Cash or Debit,), and all major credit cards. 

 Please verify insurance before your appointment. Depending upon your particular health care plan, prior authorization may be required before your initial appointment takes place. Clients are encouraged to contact their healthcare provider to determine if mental health services are covered by their policy.

Payment is required prior to treatment. Any returned checks will result in a service charge fee of $40 that is due before scheduling your next appointment.

• Preparation time (including submission of records or letters, summaries, narratives and/or reports): $325/hr
• Phone calls: $325/hr
• Depositions: $325/hour
• Time required in giving testimony: $325/hour
• Mileage: $0.54/mile
• Time away from office due to depositions or testimony: $325/hour
• All Fees must be paid in full prior to court appearance. 

 All attorney fees and costs incurred by the therapist as a result of the legal action require that payments are to be paid in full.

•  The minimum charge for a court appearance: $3,500.00

• Initial Diagnostic Interview - $165
• Individual Therapy - $130 and up*
• Family/Couple Therapy - $150
• Group Therapy - $50*

All services are non-refundable and/or transferable. All fees are due upon rendering service.
